New Patient Information

Everything you need before your first appointment. What to bring, what the visit looks like, and how billing works, so nothing catches you off guard at the front desk.

Before You Arrive

Four things to bring and one to do early. Having these ready keeps your first visit focused on your health instead of paperwork. Plan to arrive 15 minutes before your scheduled time.

Before You Arrive
  • Silhouette of a person in an avatar style, depicting a login or user profile icon.

    Photo ID & Insurance

A current driver's license or state ID, plus your insurance card if you plan to use coverage for the visit.

  • An illustration of medication. 6 pills inside their packet.

    Medication List

Everything you take, including supplements and vitamins, with dosages. A photo of the bottles on your phone works just as well.

  • A black and white folder illustration with a black handle and a cross graphic on the front.

    Prior Records

Recent lab work, imaging, or notes from another specialist, if you have them. We can also request records on your behalf.

  • Speech bubbles rotating. One speech bubble has a checkmark, the other has a question mark.

    Your Questions

Write down what you want to cover before you come in. First visits move quickly, and it is easy to forget something.

What To Expect

New patient visits run about 45 minutes because the history matters. Your physician wants the full picture before recommending anything, and that takes more than a rushed intake.

What To Expect
A person receiving a back massage from an osteopathic physician in a bright room.

Medical & OMT Patients

You will review your health history, current medications, and what brought you in. Then a physical exam, and if appropriate, hands-on osteopathic evaluation. You leave with a plan, whether that is labs, a treatment schedule, a referral, or a follow-up date.

A person lying on an operating table receiving an injection or anesthesia, with an osteopathic physician gloved hand holding a syringe near their forehead.

Aesthetics & Wellness Patients

Your first appointment is a consultation. We discuss your goals, review your health history for anything that affects candidacy, and walk through realistic options, timelines, and pricing. Treatment may happen the same day depending on what you choose.

Patient Portal

Your records, results, and messages live on the secure eClinicalWorks portal and Healow mobile app.

After your first visit, you will receive an invitation to our patient portal. Use it for lab results, visit summaries, refill requests, and non-urgent messages to your care team. Intake paperwork is completed there as well, since it keeps your medical information properly protected.

If yours is not here, call the office and ask. Nobody at the front desk will rush you off the phone, and there is no wrong question to ask before booking.
